Field Trip! I want all of the permission slips signed before we go on Spring Break!

We need 5 parent volunteers. Since we are so squishy on the bus, all parents will need to drive/carpool to the location in Logan. 

Read over the entire form. You can print, sign and bring it in. Or, just sign the Google Doc permission Slip. We are very excited about this!
