Maturation, Friday 3/13

Dear Parents/Guardian:

As part of your child’s education, he/she will be invited, along with the parents/guardians, to participate in a maturation program of instruction, which includes Health Core Standards outlined by the Utah State Office of Education. Maturation will be held on Friday, March 13th in the Q1 gym. Girls will attend at 8:35 am, and boys will be at 9:25 am. Parents/guardians/trusted adults are invited and encouraged to attend.

Please read the form carefully, select one option, and electronically sign to give your permission for your student to participate. Your student will not be allowed to participate without this completed form.
