Class Disclosure
Mrs. Parr’s 5th Grade Classroom Disclosure

 Welcome to 5th Grade! I am so excited to teach this year at Quest Academy, and get to know all of my new students! Please read this document carefully, and if you have any questions at all please let me know before signing the disclosure. Communication is vital, and it is the key to a successful school year; please feel free to contact me at any time.



Expectations

For Myself:
   o   I will be prepared to teach every day.
   o   I will keep parents and students informed through my blog.
   o   I will treat each student in a kind and respectful manner.
   o   I will laugh at my own mistakes, and grow from them.
   o   I will respond to questions and concerns in a timely and respectful manner.
   o   I will be an example to my students.
   o   I will do my very best every day, because my students deserve my best!

 For Parents:
   o   Parents will respectfully communicate with me any questions or concerns they may   have in a timely manner.
   o   Parents will take time to check my blog and email to stay informed.
   o   Parents will be involved in their child’s education by: asking questions, volunteering,   helping their child with homework and class projects (when appropriate).
   o   Parents will follow through on their child completing and turning in their homework.
   o   Parents will treat other parents, students, and school personnel with respect and   kindness.
   o   Parents will be a positive example to their child.

For Students:
   o   Students will come to class prepared, ready to learn, and will complete assignments   in a timely manner.
   o   Students will treat each other, school personnel, parents, and others with respect   and kindness.
   o   Students will keep me informed by asking questions to clarify assignments and new   concepts.
   o   Students will do their very best on every assignment; every assignment given has a purpose.
   o   Students will laugh at their own mistakes, and grow from them.
   o   Students will enjoy learning, and will do their best every day!



 Classroom Policies

Absences
Student work will be left on top of their desk with a form indicating what they missed while they were absent. It will be the student’s responsibility to meet with me to go over the material and ask questions about the work. Class work that was missed is to be completed as homework and turned in by the date specified. The number of days your student has to make up absent work is equal to the number of days he or she has missed. If you plan to pick up your student’s work at the end of the day, please notify the office by 9:00a.m. to give me time to get the assignments put together before the end of the day.

Birthdays
Administration has asked that students will not bring in any food-type treats. Please do not feel “obligated” to have your child bring in anything for their birthday. To avoid any hurt feelings, please mail birthday invitations to classmates’ homes. Each child’s birthday will be celebrated in class. Students with summer birthdays will be celebrated on their Half-Birthday!

BlogClassroom Information
This is where you can keep on top of what is happening in the classroom.  You can access my blog from the Quest Academy website, under “Faculty”. Parents will be required to follow my blog this year to receive up to date emails and notices about classroom happenings. Also, if you have any questions, feel free to send me an email or call me at the school.

Homework
Students are expected to be responsible for completing and turning in all homework and projects on time. Homework will be posted to the blog weekly and completed using their Google Classroom Account. Students can access the link for Google Classroom on my blog under the “Homework Tab.”

  • Students are expected to read at least 100 minutes a week, and log their minutes on a Google Form.
  • Students are expected to create a Prodigy account and complete math games at least 100 minutes a week. 
  • For all other assignments, students will receive 3-4 weeks notice before the due date.

Classroom Money & Store
  -Students will earn classroom money by completing weekly jobs, completing work on time, staying focused and on task, and setting a positive example in class.
  -Students will be able to buy items from the classroom store once a month. Some items include, but are not limited to: wear slippers in the classroom, bring a snack to school, extra art time, etc.



Tests & Grading Procedures

Tests
Tests are used to gauge student learning as well as my teaching.

  • Math: Students will be tested on each math standard. These tests will happen about once a week.
  • Reading: Students will be tested on the reading chapter covered in class each Friday.

Elementary Grading Scale

4      100%-90%      The student demonstrates mastery of the standard (can apply                                          and teach).

3      89%-80%        The student demonstrates proficiency of the standard.

2      79%-60%        The student is approaching proficiency.

1      59%-0%          The student is well below proficiency.

Grades will be updated on a weekly basis. You can view your child’s grades, as well as any missing assignments on the SIS program located through the schools website under the “Parent Tab”.  Be sure to check your child’s progress. You can obtain a username and password from the front office during back to school night or anytime during the year. I usually update the grade book throughout the week so Friday will be the best day to check your student’s grades.
