Field Day Form

Dear Parents;
We are finalizing our last field trip of your student’s fifth grade. This field trip will be your student’s field day activity.
On Wednesday, May 25th, the entire 5th grade will be taking the Frontrunner to and from the Roy Station to the Ogden Station. Student will need to be dropped off and picked up from the Roy Station. We will then walk down to the Ogden Junction Complex. While there, STUDENTS ONLY, will be able to participate in the IRock Wall Climbing, Flow Rider Surfing, IFly Air Tube, and bowling at FatCats.
To help us prepare for this field trip we would like to get a final count of the students and adult chaperons, who will be attending. For students we are asking for a $15 donation. If you have already paid your $30 donation from early this year, then you are good, the $15 was included in that donation. If you haven’t, we are asking for that donation to be paid to the Q1 front Office by April 29th.
If you would like to attend as an adult chaperone, please note that only the students will be allowed to participate in the activities, per The Junctions instructions. The only cost for adults will be the Frontrunner Fee of $3.75, which also needs to be paid to the Q1 front office.
We do need an accurate account of how many people, adults and students, will be attending, so we can ensure we have enough funds to cover the cost of this field trip.
Please fill out the permission slip below and pay your donations ASAP. Also, if your student will be attending, you need to fill out The Junctions’ permission slip that can be found at: www.iflyutah.com. We will send out an update in May with further details about the days schedule as well as any other amount in donations that are still needed.

Hi - My name is Mrs. Young. I am excited to start a new adventure teaching at Quest Academy. I have taught previously in Weber School District in 2, 3, 4, and 6th grades. This will be the start of my 13 year. I also taught in Ogden District, before receiving my Bachelor's of Science, in special education preschool and long term subbing for math in the Jr. High Schools. My hobbies are my family!!! AND singing and acting. I have always loved art and theater. I hope to incorporate art into the technology and science to help our young minds expand to new measures. My family has been part of the education system since I was born. I have always been taught to have a passion for learning and growing. I am very excited to help my students this year to achieve all their goals and never stop fulfilling their passions in life. Welcome to 5th GRADE!!!
