Field Day–Treat Permission Form

Field day will be Thursday, May 24th.  As part of the activities, we would like to provide the students with a cookie as a treat.  Please give permission below allowing your student to have one.  The cookies will be Loft House.

POQA will also be providing cotton candy for the students on Thursday. 🙂
